Food and specialist retail: Enquiry form for meal kit operators
An enquiry form for meal kit operators that asks for household size, dietary preferences, allergens and delivery day at sign-up, so the very first box is right.
What makes an enquiry complete for meal kit operators.
For a meal kit operator, the sign-up determines what ends up in the box. Yet the form often asks only for an address and a box size. That someone dislikes coriander, has a child with a peanut allergy or is only home on Thursdays surfaces when a complaint arrives. Comments typed into a free text field, such as please do not leave it with the neighbours, never reach the packing crew. Business requests, for instance an office wanting weekly lunch boxes, come through the same consumer form and arrive without invoicing details or a named recipient.

From form to assessed enquiry.
Ask about household and wishes
The form asks for the number of people, any children and standing preferences such as vegetarian or no fish. Allergens are chosen from a fixed list rather than typed. Those choices flow straight into menu selection, so dishes that do not suit this customer are skipped or replaced before the box is ever packed.
Record delivery day and drop spot
After the address, the form shows only the delivery days and time slots that exist for that postcode. The customer says where the box may be left if nobody is home, using fixed options rather than free text. That instruction ends up on the shipping label and with the delivery partner.
Confirm and pass on the sign-up
Before finishing, the customer sees a summary with first delivery date, box and preferences. After confirmation the sign-up goes to the subscription platform and the packing schedule. A business request follows its own route with questions about invoicing, a contact person and fixed delivery days.
Enquiries in one place, not the inbox.
Sign-up usually runs through a subscription platform that handles payments and cancellations. The new form sits in front of it or inside it and supplies structured preferences and allergens instead of a free text field. The allergen list comes from the same source as the recipe cards. Delivery days per postcode come from the delivery partner. The customer profile stays owned by the subscription platform; the packing schedule reads what it needs from there.
Detailed preferences or a quick start
Many questions up front make the first box better, but raise the chance someone gives up halfway. Few questions make sign-up fast, but the customer then has to complete their profile later. A common approach is allergens and household size required at the start, taste preferences optional and editable in the account afterwards.
Exclude allergens or just warn
You can exclude dishes containing an allergen entirely for a customer, or show them with a clear warning. Excluding is safer but narrows the choice sharply when someone has several allergies. Warning puts the responsibility on the customer. For serious allergies most operators choose exclusion plus a replacement dish.
Business customers on the same form or separately
An office ordering weekly boxes for staff has different questions from a family: billing address, purchase order number, multiple delivery addresses. One form with a branch keeps things tidy; a separate form makes it simpler for both groups. The choice depends on how many business requests you actually receive.
How it already runs at similar businesses.

Read the storyWhich questions the form asks.
At the start the sign-up form asks for people, preferences, allergens, delivery day and drop spot and passes them in a structured way to the subscription platform and the packing schedule. With you we practise an ordinary sign-up from a couple, and a family in which a child has several allergies, and we check that the packing crew sees the chosen wishes. A separate business route and changing preferences afterwards in the account can be switched on after that.

Explore pricing and optionsCan a customer change their preferences later?
Yes. In their account a customer can change preferences and allergens. A change applies from the next week whose menu selection is still open. If that week has already closed, the customer is told clearly, so they do not expect tomorrow's box to be different.
How do we prevent typos in allergens?
By not letting people type them. The customer picks from a fixed list that matches how allergens appear on the recipe cards. If someone has an allergy not on the list, the sign-up goes to a member of staff before the first box is packed.
What about addresses outside our delivery area?
The form shows straight away that the postcode is not served and offers to keep the address for when you expand. The sign-up does not continue to the subscription platform, so no payment starts for a box that cannot be delivered.
Can the form handle a gift subscription?
Yes, with a separate choice at the start. The form then asks for the recipient, their address and preferences if known, and for the giver's details for payment. The recipient gets the right box and the giver gets the invoice.
